To register for Puppy Kindergarten please download and print the Puppy Packet which includes the following:                

• Puppy Kindergarten Information Sheet
• Puppy Kindergarten Registration/Responsibilty Waiver Form
• Puppy Vaccination Form 
• Credit card/debit card form

To register for Beginning Obedience Class please download and print the Obedience Packet which includes the following:                

• Beginning Obedience Information Sheet
• Beginning Obedience Registration/Responsibilty Waiver Form
• Over 6 months old Vaccination Form
  
• Under 6 months old Vaccination Form
  
• Credit card/debit card form
